Records the decisions on the live-upgrade design (#5):

• production keeps deploying via pixienix + nixos-rebuild switch; restarts are made harmless instead
• no next/shadow instance or release branch for now; the NixOS upgrade VM test is deferred with it
• webportal is out of scope

and replaces the rollout section with the wave plan being implemented (wave 1 in parallel: verdict integrity, protocol handshake, forge outbox + reconciliation, CP restarts; then acknowledged delivery; then the run supervisor).
